The following summarizes the suggested donning and doffing procedures. Further review of this protocol by infection disease specialists or EH&S officers in hospitals is required.

Sequence for putting on Pneumask and PPE.

1. Eat, drink and go to the restroom. Check that everything that you need to bring into the patient’s room is available.

2. Tie up your hair and remove unnecessary accessories on wrists, neck and ears.

3. Perform hand hygiene.

4. Put on a surgical cap, tucking loose hair away from the face and neck.

5. Put on the first layer of gloves.

6. Put on your gown by placing your head through the top of the opening and insert your arms into the sleeves. Tie the gown properly.

7. Carefully inspect the disinfected snorkel mask, adapter and viral filter of Pneumask.

8. (Optional if they are connected already) Serially connect the adapter and viral filter to the snorkel mask.

9. (Optional for Bluetooth user) Turn on the Bluetooth microphone. Check the connection with the smartphone. Put the Bluetooth microphone in a clean, small Ziploc bag. Tape and secure the Ziploc bag inside the mouth chamber without blocking the chin valve.

10. Perform a suction test: put the mask to your face, press it slightly while inhaling to create suction between the mask and your face. The mask should suction to your face. Try smiling and see if the movement of facial muscle breaks the seal. IF MASK DOES NOT PASS THE SUCTION TEST, THE MASK SHOULD NOT BE USED.

11. Put on the mask fully. Adjust and tighten the straps as needed.

12. Inhaling or exhaling while closing off the top of the filter with your hand can be done for another seal check.  With a good facial seal, the mask should suck down on your face with a good seal with inspiration, and not lift off face with exhalation.

13. Perform hand hygiene.

14. Place your gloved thumbs through the loops of the gown. Put on the second layer of gloves.

7. Inspeccione cuidadosamente la máscara de snorkel desinfectada, el adaptador y el filtro viral de Pneumask.

8. (Opcional si ya están conectados) Conecte en serie el adaptador y el filtro viral a la máscara de snorkel.

9. (Opcional para usuarios de Bluetooth) Encienda el micrófono Bluetooth. Verifique la conexión con el teléfono inteligente. Coloque el micrófono Bluetooth en una bolsa ziplock pequeña y limpia, pegue y asegure la bolsa ziplock dentro de la cámara oral sin bloquear la válvula de la barbilla.

10. Realice una prueba de succión: colóquese la mascarilla en la cara, presiónela ligeramente mientras inhala para crear una succión entre la mascarilla y su cara. 12. Se puede inhalar o exhalar mientras se cierra la parte superior del filtro con la mano para realizar otra verificación de sellado. Con un buen sellado facial, la mascarilla debe absorber su rostro con un buen sellado con la inspiración y no despegarlo con la exhalación.

13. Realizar higiene de manos.

14. Coloque sus pulgares enguantados a través de los lazos de la bata. Ponte la segunda capa de guantes. (ver video)

Instrucciones para quitarse la máscara:

Sequence for taking off Pneumask and PPE.

1. In the anteroom (or the doorway if there is no anteroom), grasp the gown and pull away from your body. Carefully fold the gown into ball-shape and only touch the outer surface of the gown with gloved hands. Remove the gown and the outer gloves together. Dispose of the gown and the gloves into a dedicated trash can.

2. Perform hand hygiene (you should still be wearing your inner layer of gloves).

3. Leave the anteroom.

4. Perform hand hygiene.

5. Inspect the integrity of the inner glove. If the inner glove is intact without gross contamination, proceed through the next step. If the inner glove is torn, broken, or grossly contaminated, remove the inner glove. Perform hand hygiene, and put on a new pair of gloves.

6. Lean head forward into sniff position. Loosen the straps and remove the mask without touching your face.

7. Use EPA-approved alcohol-based wipe, bleach wipe, AF3 wipe or hydrogen peroxide wipe to wipe the outer surface of the mask, adapter and filter. Dispose of the wipe. The required contact time for different EPA-approved wipes can be found in Appendix A Table 5.

8. Use another EPA-approved alcohol-based wipe, bleach wipe, AF3 wipe or hydrogen peroxide wipe to wipe the inner surface edges of the mask. Dispose of the wipe.

9. Gently disconnect the filter from the mask. Dispose of the filter if it is designed for one-time usage. If you plan to reuse the filter, follow the instructions from the manufacturer of the filter to store it properly, and DO NOT WET the surface of the filter.

10. Put the mask into a clean dedicated box.

11. Perform hand hygiene.

12. Remove the surgical cap. 

13. Perform hand hygiene.

14. Remove the inner gloves.

15. Perform hand hygiene.

16. (Optional for Bluetooth user) Remove the Ziploc bag which contains the Bluetooth microphone. Wipe the surface of the Ziploc bag with EPA-approved wipe (Appendix A Table 5) and open the Ziploc bag. Perform hand hygiene. Dump the microphone from the Ziploc bag with one hand (dirty hand) onto the other hand (clean hand). Dispose the Ziplock bag with your dirty hand. Place the microphone somewhere you will not forget with your clean hand. Perform hand hygiene.

17. By the end of the shift, bring the box and the mask to somewhere you can fully decontaminate it following our decontamination protocol, or hand it to hospital technicians per hospital policy. The recommendation of using disinfection wipe between patients and fully washing it after one shift is following the recommendation of NIOSH on elastomeric respirator.

5. Inspeccione la integridad del guante interior. Si el guante interior está intacto sin contaminación grave, continúe con el siguiente paso. Si el guante interior está desgarrado, roto o muy contaminado, quítese el guante interior. Realice la higiene de las manos y póngase un par de guantes nuevos.

6. Incline la cabeza hacia adelante hasta la posición de olfateo. Afloje las correas y retire la mascarilla sin tocar su cara.

7. Utilice un paño a base de alcohol, un paño con hipoclorito, un paño AF3 o un paño con peróxido de hidrógeno aprobados por la EPA para limpiar la superficie exterior de la máscara, el adaptador y el filtro. Deseche la toallita. El tiempo de contacto requerido para diferentes toallitas aprobadas por la EPA se puede encontrar en el Apéndice A, Tabla 5 en nuestro documento abierto.

14. Quítese los guantes interiores.

15. Realice la higiene de manos.

16. (Opcional para usuarios de Bluetooth) Retire la bolsa con cierre hermético que contiene el micrófono Bluetooth. Limpie la superficie de la bolsa ziplock con un paño aprobado por la EPA (Tabla 5 del Apéndice A en nuestro documento abierto) y abra la bolsa ziplock. Realice higiene de manos. Vierta el micrófono de la bolsa ziplock con una mano (mano sucia) en la otra mano (mano limpia). Deseche la bolsa ziplock con la mano sucia. Coloque el micrófono en algún lugar que no olvide con su mano limpia. Realice higiene de manos.

17. Al final del turno, lleve la caja y la mascarilla a un lugar donde pueda descontaminarlas por completo siguiendo nuestro protocolo de descontaminación, o entréguelas a los técnicos del hospital según la política del hospital. La recomendación de usar una toallita desinfectante entre pacientes y lavarla completamente después de un turno sigue la recomendación de NIOSH sobre respiradores elastoméricos.

Disclaimer: This document summarizes the proposed cleaning and decontamination protocol for the snorkel mask and adapter. Please note that the following guideline is based on the OSHA procedure for cleaning respirators (it has not been approved for this virus or these snorkel masks). Further review of this protocol by infection disease specialists or EH&S officers in hospitals is required.

1. Perform hand hygiene.

2. Put on a gown, gloves, and a protective mask.

3. Wipe the surface of the filter with 70% ethanol or hydrogen peroxide, and carefully remove the filter.

4. Discard the wipe as biohazardous waste. Discard the filter if it is intended for single-use only. If you plan to reuse the filter, follow the instructions from the manufacturer of the filter to store it properly, and DO NOT WET the surface of the filter.

5. Wash mask and filter adapter thoroughly in warm water (43℃ [110℉] maximum) with a mild detergent. A sponge that won’t scratch may be used to facilitate the removal of dirt. Make sure to clean all the valves to avoid potential clogged valves.

6. Rinse the mask and adapter thoroughly in clean, warm (43℃ [110℉] maximum), running water. Drain.

7. Prepare a 50ppm chlorine solution: add one milliliter of laundry bleach per liter of warm water (43℃ [110℉] maximum). Prepare enough solution to fully immerse the entire mask. Remove all the air in the side channel to ensure full immersion. Please note that mixing bleach solutions with detergents can generate toxic substances. Make sure to wash out all the detergents before immersing the mask and adapter into bleach.

8. Immerse the mask and adapter in the hypochlorite solution (50 ppm of chlorine) for 2 minutes.

9. Wearing fresh gloves, rinse the mask and adapter thoroughly in clean, warm, running water (43℃ [110℉] maximum). The mask and adapter must be thoroughly rinsed with water to remove any detergents or disinfectants that may result in dermatitis or damage of the mask components.

10. Dry the mask and adapter with a clean lint-free cloth or allow to air dry.

11. Reassemble the mask and store in a clean space.

Las siguientes instrucciones se basan en las instrucciones de OSHA sobre la limpieza de un respirador. 6. Enjuague bien la mascarilla y el adaptador con agua corriente limpia y tibia. Desagüe.

7. Prepare una solución de hipoclorito de 50 ppm: agregue un mililitro de blanqueador para ropa por litro de agua tibia. Prepare suficiente solución para sumergir completamente la mascarilla.

8. Sumerja la mascarilla y el adaptador en una solución de hipoclorito (50 ppm de cloro) durante 2 minutos.

9. Con guantes nuevos, enjuague bien la mascarilla y el adaptador con agua corriente limpia y tibia. La mascarilla y el adaptador deben enjuagarse a fondo con agua para eliminar cualquier detergente o desinfectante que pueda provocar dermatitis o daños en los componentes de la mascarilla.

10. Seque la mascarilla y el adaptador con un paño limpio que no suelte pelusa o deje secar al aire.

11. Vuelva a montar la mascarilla y guárdela en un espacio limpio.

It is imperative for each clinician attempting to use Pneumask in a clinical setting to first conduct a qualitative fit test with a trained administrator. These would be done with the full assembly (snorkel mask, adapter, and bacterial/viral filter) while performing a series of movements. Please see video below for an example of one of these tests.

Additionally, before each shift or use, please perform the basic positive/negative seal test to ensure proper donning and fitting. These are outlined as part of the donning instructions.

Wearing a full-face snorkel mask may inhibit the user’s ability to communicate by muffling the user’s voice. We have noted this phenomenon and have developed an app that may help alleviate the issue. The app will take the audio feed from a Bluetooth device with a microphone (such as AirPods or the like) placed inside the mask and replay it on the phone’s speakers outside the mask.

Android

We have tested the Android version on several phones, and it works fine on some while others experience issues. (This is due to different device manufacturers handling audio routing differently). We are working on compiling a list of verified devices, but in the meantime, you’re free to download the app from the Google Play Store and try it out.

iPhone

Currently iOS does not allow using two audio devices at once. We are actively addressing this issue with insiders at Apple, and we hope to have a working version on the App Store soon.

Amplificación de voz

El uso de una máscara de snorkel que cubra todo el rostro puede inhibir la capacidad del usuario para comunicarse amortiguando la voz del usuario. Hemos observado este fenómeno y hemos desarrollado una aplicación que puede ayudar a aliviar el problema. La aplicación tomará la señal de audio de un dispositivo Bluetooth con un micrófono (como AirPods o similar) colocado dentro de la máscara y la reproducirá en los altavoces del teléfono fuera de la máscara.

Android

Hemos probado la versión de Android en varios teléfonos y funciona bien en algunos, mientras que otros experimentan problemas.
